The Arizona Department of Health Services - Bureau of Vital Records's Office is the central office for the state of Arizona and it issues certified copies of Arizona birth certificates, Arizona death certificates and Arizona marriage records for events that occurred in Arizona. You may order copies of Arizona vital records through this office or, in some cases, the office of the county clerk where the deed happened.

Arizona is a state located in the southwestern region of the United States. It is best known for being the home to the Grand Canyon—one of the world’s seven natural wonders.

The state of Arizona became the 48th U.S. state on February 14, 1912. It has a population of 7,151,502, according to the census estimates of 2020. 

Arizona’a capital and most populous city is Phoenix; Tucson, Mesa, Chandler, and Gilbert are among other major cities. The state is divided into 15 counties, the main ones are Maricopa County, Pima County, Yavapai County, Mohave County, and Yuma County.

Birth and death records from 1909 onwards are kept by the Bureau of Vital Records, a division of the Arizona Department of Health Services. The office also has abstracts of records that were filed before this time.
