Montezuma County Vital Records

The official processing time for a vital record in Colorado is 21-25 days. You are advised to ensure you allow the maximum time for your application to be processed.

To avoid any delays in the processing of your birth certificate request, make sure that your application meets all requirements and that there are no mistakes on your form.

The price of a certified copy of a birth record in Colorado in $20. You will still be charged the fee if no document is found after searching the archives.

For each additional copy of the same document, there is a $13 fee.

Payments should be made to the Vital Records Section, and can be made by check or money order.

You must meet the document requirements to apply for a vital record request through the Office of the State Registrar of Vital Records in Colorado. This includes a completed application form, and a valid ID document.

Acceptable forms of ID include: 

• Passport

• Driver’s license

• Photo ID card

• Government work ID

• Permanent resident card

• Certificate of U.S. citizenship

If you do not have an ID from the primary list, you may show 2 forms of ID from the secondary list. These include:

• U.S. Birth certificate 

• Court order of adoption or name change

• U.S. divorce decree

• U.S. marriage license

• Property tax receipt
