The Department of Health in Honolulu processes most vital records requests within 6-8 weeks. Please allow the maximum time for your application to be reviewed and approved.

Please check your application against the instructions provided on this page. Any mistakes on your form could result in delays, or even a rejected request. The same applies to any missing documentation that you fail to provide.

An official birth certificate copy in Hawaii costs $10. You must pay the fee when you submit your application with the state Department of Health.

There is a $4 fee for each additional copy of the birth record that you request. There is then a $2.50 charge added for every 5 copies. 

If you order your birth certificate copies online you will need to pay with a debit or credit card, which must match the name of the applicant. Mail orders can be paid by cashier’s check, certified check, or money order. 

For in-person applications you can pay using any of the payment methods mentioned above, including cash.

In addition to a completed vital record application form, you must have a government-issued photo ID to confirm your identity.

Accepted forms of ID include:

• State-issued ID card

• Driver’s license

• Passport

If you are not the named person or one of the parents on the birth certificate, you must also have a document proving you are entitled to a copy. Entitled persons include: 

• Spouse

• Child

• Grandchild

• Legal guardian

• Personal representative
