The document requirements for a vital record in Idaho include: a completed application form and an accepted identification document.

All applicants must have 1 of the following forms of government-issued ID:

• Driver’s license

• State ID

• Passport

• Concealed handgun license

• Offender ID card

OR 2 of the following:

• Employment Authorization Document (EAD)

• School ID card

• Medical insurance card

• Car insurance card

• Veterans Affairs card

• Vehicle registration

• Paycheck stub with imprinted information

If you are not the named person on the record, you must also have paperwork to prove you have “direct and tangible interest” to request the document.
