The Indiana Department of Health—Division of Vital Records has an expected processing time of 12-16 weeks for vital record copies.

If you were born in Indiana, you can place your order through your local county’s health department for faster processing.

To avoid delays with your birth record application, you are advised to ensure that you complete your application correctly. Any mistakes or missing information could result in a rejected request.

A certified copy of a birth record in Indiana is $10. The payment can be done by check or money order, payable to the Indian Department of Health.

For additional copies of the same birth certificate, there is a $4 charge per document.

Please note that once you have submitted your application, the fee is non-refundable.

To avoid paying for additional applications, you are advised to check your application carefully before you submit it.

You must have a completed application form and a document proving your identity to be eligible for a copy of a vital record.

Acceptable form of ID include:

• Driver’s license

• State ID

• Military ID

• Passport

• School ID

• Work ID

If you do not have one of the primary forms of ID, you may show 2 secondary IDs. These include:

• Verification of employment

• Voter registration

• Current vehicle registration

• Previous year’s tax return

• Signed mortgage

If you are not the individual or a parent listed on the birth record, you must also show proof of your relationship to the individual.
