Kentucky (KY) Vital Records

The Office of Vital Statistics in Frankfort takes 3-6 weeks to process vital record applications. 

To ensure that your birth certificate request is processed within the expected time frame, it is essential to follow the steps outlined by the Department for Public Health. 

If you have any queries about a request you have already submitted, please contact the office where you applied.

The processing fee for a certified copy of a birth record in Kentucky is $10. This fee is nonrefundable and will be kept even if your document is not found.

Payments should be made by check or money order, payable to the Kentucky State Treasurer.

In addition to a completed application form, you must also prove your identity when you apply for a birth certificate copy.

You must have either a driver’s license or a state ID. If you do not have either of these forms of IDs, you can show 2 of the following:

• Passport

• Military ID card

• Voter registration

• Passport

• Vehicle registration

• Bank statement

• Medical insurance card

• Card insurance card

If you are not the individual on the birth record you are requesting, you must also have a document proving that you are a parent or legal guardian.
