Requesting a certified copy of a New Jersey birth record requires the payment of a fixed charge. 

You will be required to pay $25 for the initial copy of a New Jersey birth certificate that you order.

If you order any additional copy of a birth certificate in your order, you will need to pay $2 for each extra copy in your request. 

You can submit a check or money order that must be submitted to the New Jersey General Treasury.

To ensure the correct processing of your application, you must include a photocopy of your identification that is valid in New Jersey. 

In New Jersey, you must submit just one of the following forms of primary ID: 

• State driver’s license or non-driver’s license

• State driver’s license without a photo and an alternate identification

It is also possible to present 2 of the following alternative forms of identification, instead of one primary identification: 

• W-2 form or tax return for the current or past tax year

• Federal or state identification

• U.S. or foreign passport

• Bank statement (less than 90 days old)

• Voter registration

• School identification

• Utility bill (less than 90 days old)

• Immigrant visa

• Green card

• Vehicle registration or insurance card

Ensure these documents are not expired to easily legitimize them as valid forms of ID.
