The total amount for a copy of a birth certificate in Utah will depend on your order. The initial copy of a Utah birth certificate is $20. 

If you include additional copies in the same order, you will need to pay $10 for each extra copy that you request. 

You can choose to pay the fee either by check or money order made payable to Vital Records. 

If you do not include the correct amount on your check or money order, your order will face delays.

A valid form of identification is required with your birth certificate application in Utah. Only certain forms of ID will be accepted. 

You will need to photocopy the photo identification(s) that you select. 

You may submit a photocopy of one of these photo IDs:

• Driver’s license

• State ID card

• Current U.S. or foreign passport 

• Military ID card

• School ID 

• Green card

• Employment Authorization Document(EAD)

• Concealed handgun license

• Certificate of Naturalization or Citizenship
