You must pay a set fee to receive your birth certificate copy in Vermont. It’s essential to include the payment with your application, or your birth certificate will take more time to process. 

For each copy of the certificate that you order, you must pay a fee of 10 dollars for a certified copy, either by personal check or money order. 

The Personal check or money order must be made payable to the Vermont Department of Health.

To prove your identity to the Vermont Department of Health, you must submit an acceptable identification document.

You may submit a photocopy of one form of state identification, such as a:

• Driver’s license

• State identification card

It will also be possible to submit photocopies of 2 of the following documents: 

• U.S. Military ID

• Voter registration card

• Vehicle registration

• Bank statement

• U.S. Passport or a foreign passport

• Medical or car insurance card
