Copies of birth records in the state of Washington may be requested by the individual named on the certificate or by the parent or legal guardian of the individual on the certificate.

The fee in Washington is fixed at an initial price of $25 for a copy of the vital record.

The acceptable forms of payment for a birth certificate in Washington include a check or money order. Either form of payment must be made payable to the State Health Department.
