If you order additional copies at the same time, it will be necessary to pay $3 for each extra copy that you request. 

Paying the incorrect amount will delay your birth record request. 

You may pay the fee either by check or money order made payable to the State of Wisconsin Vital Records.

To apply for a Wisconsin birth certificate, you need to also submit an acceptable form of identification. 

You can submit a photocopy of one of the following primary ID documents: 

• State driver’s license or 

• State photo ID

If you are unable to provide a primary form of identification, you can submit photocopies of any 2 secondary forms of ID, such as: 

• Utility bill

• U.S. or foreign passport 

• Bank statement or pay stub 

• Lease/rental contract

• Traffic ticket

• Medical/Medicare insurance card
